Summary

Collaboration among healthcare providers is critical to making diagnoses in children with undiagnosed complex medical conditions. Brenner Children’s Hospital developed an innovative program called the Declan Donoghue Collaborative Care Program in honor of Declan Donoghue who died of a rare medical condition that was undiagnosed prior to his death.

The goal of this program is to increase the collaboration among healthcare providers in an effort to make diagnoses in children with undiagnosed complex conditions.

In this session, Ms. Nicole Donoghue and Dr. Savithri Nageswaran will describe why and how the Declan Donoghue Collaborative Care Program was developed, the model of collaborative care and clinical outcomes of the program.
